The AstroCamp camper checklist of required camp forms for registration helps parents prepare their child for summer camp. It includes camp tuition and fees, camper experience, camper code of conduct, transportation, camp store account, the parent authorization form, health history form, and physician’s exam. All the required forms and the full tuition are due by May 1st. If you register later than May 1st, you must pay the full tuition and all forms must be submitted as soon as possible.
